Medication Administration

KP MedRite — Completed 2007

As of 2006, there were approximately 7000 deaths per year in the United States due to medication errors. This amounts to about one medication error per patient per day.

In 2007, Kaiser Permanente's National Quality, KP HealthConnect, National Patient Care Services, and the Innovation Consultancy (IC) collaborated to tackle this tough issue.

We learned that the medication administration process is unclear, chaotic and full of interruptions. With nurses as co-designers, we developed an evidence-based design that is simple, elegant, and safe. We call this innovation KP MedRite.

The final set of solutions is designed for the clinic visit:

Our two pilot sites in West Los Angeles and Hayward showed improvement after introducing our solution set.

  • 50% reduction in the number of staff interruptions to the medication administration process
  • 15% faster per med pass
  • 18% increase in on-time med passes
  • Increase in process reliability from 33% to 78%

In addition, there was a 105% ROI for the project in 2 years, and KP MedRite was cited as “Good Practice” by JCAHO.
